Description
Every Ambulance Service must apply for and receive an ambulance vehicle license from the Oregon Health Authority (OHA-EMS) before offering such service to the public.
Duration
Annually, initial license not to exceed 15 months.
Renewal Information
Shall be valid until June 30 of each year, unless sooner revoked or suspended. License is non transferable.
