Required Documentation

Errors and omissions insurance of $1,000,000 with the firm named as the insured. An Attestation of Errors and Omissions Insurance form is available at https://dfr.oregon.gov/business/licensing/financial/Documents/5379.pdf and is required to be submitted with the policy declaration page. Renewal or change of the policy requires submission of correct information to DFR.
