Review the Training Guide to Performing IMEs (4913) or a WCD approved vendor training at https://wcd.oregon.gov/Publications/4913.pdf
Pass the online quiz at https://dcbs.qualtrics.com/jfe/form/SV_em3bvdl10QEPIFf
Complete the IME application at https://www4.cbs.state.or.us/exs/wcd/ime_app/index.cfm
The workers’ compensation division will inform the provider when they have been added to the authorized IME provider list.
