Description
A Retail Drug Outlet for Home Dialysis supplies may provide dialysis solutions under the general supervision and direction of a pharmacist with special training in renal disease and dialysis to end stage renal disease (ESRD) patients who have chosen the option of home dialysis therapy and who have been appropriately trained.

Required Documentation
Submission of an application on the form prescribed by the State Board of Pharmacy and applicable fees as required in OAR 855-110. See application for detailed list of required documents, which includes: a copy of resident state license/registration and verification for all non-resident applicants and a legible 8.5” x 11” floor plan, which identifies the location of drug storage, sinks, refrigerators, windows and doors. Windows and doors must be marked as secured or unsecured.
Registration with the Oregon Secretary of State's Business registry is also required.
