Renewal Information
Renewals are processed in the same manner as the initial application. For additional information, please contact the Commission Licensing Office at (971) 352-0455.
Duplicate License Requirements
If a license is lost or destroyed, a replacement fee of $10 may be required for a duplicate license. Duplicate license expiration date is the same as the original license.
