This Certificate is for those agencies that contract with the State or county to provide community mental health outpatient services.
Duration
Up to three years.
Renewal Information
A desk audit is conducted upon receipt of application followed by a site review. Contact the Office of Mental Health and Addiction Services at 503-945-9700 to request the application.
