Description
A person who has received formal training in prehospital and emergency care, and is licensed to attend any person who is ill or injured or who has a disability. Police officers, firefighters, funeral home employees and other persons serving in a dual capacity one of which meets the definition of "emergency medical services provider" are "emergency medical services providers" within the meaning of ORS 682.
Duration
Biannual, on odd-numbered years.
Renewal Information
Shall be valid until June 30 of each odd-numbered year, unless sooner revoked or suspended. The initial licensing period may not exceed 30 months.

Prerequisites and Requirements
Prerequisites
Must be at least 18 years of age, submit proof that the applicant received a high school diploma or equivalent or a degree from an accredited institution of higher learning.
Required Documentation
Completed application, non-refundable fee, submit proof of successfully completing an approved course, submit proof of passing the required cognitive and practical examinations, consent to a criminal background check, meet all requirements for that level as established in OAR 333-265-0000 through 333-265-0023; Demonstrate proof of current National Registry certification, and make application within 24 months from the date that their training program was completed, unless an applicant has been on active duty in the military within the last four years and in that case, the application may be submitted more than 24 months from the date the training program was completed.
Continuing Education Requirements
Complete 36 hours of continuing education as specified in Appendix 1, incorporated by reference.

A license will be issued within three (3) weeks of successful examination.
